Rates
$135 per 45-min session
Clients who chose to pay directly for services can make payment via credit or debit card at the time of service. I am not seeing new patients under the age of 18, couples or families.
Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. I am an in-network provider for the following payers:- Aetna
- Allied Benefits
- Bureau of Worker's Compensation (BWC)
- Frontpath
- Medical Mutual
- Medicare
- Paramount
- Parkview Signature Care
- Tricare
I am not a Medicaid provider. I am also out-of-network for Anthem, Blue Cross Blue Shield, United Healthcare and Cigna.
Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- Is Dr. Davies an in-network or out-of-network provider?
- If Dr. Davies is an out-of-network provider, can I still receive coverage?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
If Dr. Davies is not an in-network provider for your insurance carrier, you may ask your insurance company for a single-case agreement which may allow coverage.
Reduced Fee Reduced fee services are available on a limited basis.
Payment Cash, check and all major credit cards accepted for payment.
Cancellation Policy If you do not show up for your scheduled therapy appointment, and you have not provided notification at least 24 hours in advance, you will be required to pay the full cost of the session.
